COST $50.00 (subject to change)

Call the office nearest you for their preferred method of payment - which may range from cash, money orders, credit cards and debit cards, with proper identification. Certified copies of a marriage license are $18.50 (subject to change), would recommend purchasing two of these to have for changing your name.

REQUIREMENTS

1. Both the bride and groom must be present to apply.
2. No physical examination is required.
3. No blood test is required.
4. If you are under the age of 18, you must either have a notarized parental consent form or have your parents accompany you, present the proper identification, and sign the parental consent form in front of the clerk issuing your license.
5. If you are age 16 - 17 identification and one of the following documents showing proof of age is needed:
   
   • certified copy of birth certificate
   • current driver's license
   • state or military I.D. card
   • current passport.
6. If you are age 15 or under, you must also have a court order. To obtain a court order you must contact Conciliation Court at 602.506.3296.

WAITING PERIOD & EXPIRATION OF LICENSE

1. No waiting period, you will receive your marriage license and may be married on the same day you apply for it.
2. Any unused marriage license expires one year from the date of purchase.
